Youth Ministry Resources

Youth Ministry is tough, but you don’t need to go it alone. Below is a collection of resources to help with some of the common needs of youth ministry. However, if you need specific help or support with any aspect of your ministry, please don’t hesitate to contact Mike Armstrong, the Director of Youth and Young Adult Ministry for the diocese.

Diocesan Policies and Safe Environment

All adults working with minors or vulnerable adults must complete safe environment training, including a background check, every five years. All Safe Environment training and certification is done through Catholic Mutual Group. Those resources can be accessed on their website here: CMG Connect- Lexington

Liability Forms

Liability forms are required for any parish or diocesan activity that takes place off parish or diocesan property. Note that these liability forms are not required for a youth to participate in youth programming on parish grounds, such as a weekly youth group.

Below is a description of each type of form we require for various needs. Forms can be downloaded below.


To Be Completed Annually (or as needed)

Medical and Emergency Information: The Medical and Emergency Information form needs to be completed annually or as information changes; a new form does not need to be completed for every event. The original of this form must be kept in the parish office, but copies should be made available and on hand for any parish activities involving youth.

Photography and Video Consent Form: Images or video of minors cannot be included in social media and other forms of advertising without explicit written permission. Please note that a youth’s participation in an event or program cannot be contingent upon the completion of this form. If the family does not want their youth included in pictures or video, the youth must still be allowed to participate in the church activity/event, and it is the responsibility of the parish to find ways to keep that youth’s image from appearing in social media and advertising.

Transportation Form: This form is to be completed by any adult who is driving youth as part of a parish activity.

To Be Completed for Each Activity/Event That Takes Place Off Church Property

Parish Activity Form: This form should be filled out for every youth for every event, and kept in the parish office. The PDFs are fillable and it is recommended that youth ministers/coordinators fill out as many of the details as they can for families.

Adult Liability Waiver: This form must be completed by an adult if they are participating in an activity. Note that simply being present for an event, such as when acting as a chaperone, does not necessarily constitute participation.

Renewing the Vision

In 1997 the USCCB published this groundbreaking document on youth ministry outlining the three goals, 7 themes, and 8 components of comprehensive ministry with young people. 1997 might seem like a dated document for such a rapidly changing ministry setting, but the solid foundations laid in this document are still instrumental in how we approach youth ministry today.

Listen, Teach, Send

Written to accompany Renewing the Vision, Listen, Teach, Send was published by the USCCB in 2024 to create a national framework on evangelizing and accompanying young people in their journey of faith. Included in the prologue is a letter written directly to the young people of our Church, encouraging them to see themselves as the protagonists in their own jounrye with Christ.

Christus Vivit

It’s not often that popes write apostolic exhortations directly to and for young people, so its worth paying attention when one does. This exhortation was written by Pope Francis in 2019 to encourage both young people and the Church at large to see youth Catholics not only as the Chruch of tomorrow, but also as the “now of God”.
