Diocesan Policy requires all parishes and schools in the diocese of offer a Safe Environment program to students on an annual basis. Schools may choose to integrate Safe Environment as an on-going component to their exisiting curriculum however for diocesan reporting purposes to the USCCB for our annual Safe Environment Audit records each location must submit to the Chancellor's Office the following forms:

  1. Program Participation Summary
  2. Parental Opt Out Form- According to Article 12 of the Dallas Charter, the parish or school must offer to provide Safe Environment training materials to parents/guardians to utilize when they decline the child's participation in the training program offered at the location.
